Admit to: INPATIENT MEDICINE WARD UNDER THE SERVICES OF DR. HERNANDEZ

Diagnosis: SYMPTOMATIC ANEMIA

Condition: OF CARE; PROGNOSIS GUARDED

Vital signs: q4h. Call physician if pulse >90,<60; BP >150/90, <90/60; R>25, <12; T >38.5/C.

Activity:Bed rest with bedside commode.

Nursing: INPUT AND OUTPUTS CHARTED.

Diet: REGULAR DIET 2000KCAL.

IV Fluids: 0.9 NSS 1000ML AT RATE OF 125CC/HR

Medications:

-CROSS AND MATCH 4 UNITS PRBC TO INFUSE AT RATE OF 80ML/HR. CHECK HEMOGLOBIN LEVEL AFTER TRANSFUSSION.

-PEPCID 20MG IV Q12H.

-ZOFRAN 4MG IV Q6H PRN IF NAUSEA OR VOMIT.

LABS/IMAGING: CXR, CBC, blood culture and sensitivity, reticulocyte count, BLOOD TYPE AND CROSS MATCH, CMP, UA, urine culture and sensitivity, TSH AND FREE T4.

TOMMORROW AM REPEAT: CBC, BMP, FERRITIN.
